The Grosvenor Practice can complete a wide range of medical forms, reports, and certificates, including:
DVLA medicals, taxi and bus driver medicals
Shotgun and firearms certificates
Adoption medicals
Insurance reports
Return-to-work notes
Fit-to-fly certificates and sick notes
Amongst other formal reports and certificates
All forms and reports completed during a 30-minute appointment are included in the appointment fee.
Any administrative work carried out by our GPs outside of the appointment time, such as sick note extensions or amendments to reports and forms, will incur an additional charge.
Most forms and reports can be completed within a 30-minute appointment, including during the consultation itself. However, some may require additional time, and others may not be possible for us to complete at all — particularly certain types of visa medicals.
Please do not book an appointment for a medical report without contacting us by email first. If possible, email a copy of your form or report to [email protected]. We will aim to respond within 24 hours to confirm whether we can complete it and to arrange an appropriate appointment.
